Access Violation in module oleaut32.dll

I got this message the second time I launch a particular form
"Project mat.exe raised exception class EAccessViolation with
 message 'Access violation at adress 779D9405 in
 module 'oleaut32.dll'. Read of ...."
Please what could have gone wrong.
