Sure you can. But you need to worry about it. Use AntiSpyware tools. Set them up, keep them up to date. Sure, you can set that up automatically with most software. But you still need to keep and eye on it. Annoying firewall warnings. You don't even have to worry about any of this with a Mac. It just works. And not having to worry about any of this is a big plus.
