To answer my own question, I found this one to be the best of them all:

(the GUI is a little funky, but the functionality is excellent, and it's only 49 bucks)
The reason I think it's the best is because it is completely compatible with MS HTML Help Workshop, so it is a piece of cake to decompile a large .chm and merge it with your own.
Help and Manual and the other failed big time on decompiling a .chm and keeping the formating 100% accurate. I would have spent 50 hours or more fixing the formating in Help and Manual.
FAR HTML will even take the MS HTLM Workshop project and create a super nice uncompiled html help for use on the web. see this example:
