Afaik it meets all your requirements, is free and
much better than a lot of non-free tools around.

never actually had a need until now, I haven't looked at it in detail.

Team Coherence tends to get a good rap as well.

And if you have Delphi Enterprise or above, you could try StarTeam.

Then there's also JediVCL, but the server is a pain to configure (IMO) for
SQL Server.
