Help & Manual is very good. However, if you only need to create CHM and HTML help (not PDF, DOC, etc), you can get HelpNDoc, which is outstanding and easy to use, for free (non commercial) or *very* inexpensive for commercial use.

We've used it for years, it works well. You can use it in your automated builds, pass in variable values, etc. Works very well and easy to use.
