This may not be exactly what you are looking for, but certainly worth
exploring I think.
1. We have a PDF Viewer in PDFtoolkit, where you can open existing PDF
documents and (with some programming from your side) you can provide the
user with controls to drop on to the viewer using the event on the viewer
and then use the PDFtoolkit API to persist the form controls into the
PDF.

2. Perhaps a much easier one (which I just realize as I write) would be
to use the Gnostice Print2eDoc SDK, fire print job of any blank document
(or an existing Word, RTF or other template) and then allow the user to
use the full-fledged, built-it Previewer-Designer to add form fields,
buttons, annotations, text, links... and finally hit Save to save the
work as a live PDF form.
