Capturing sound card output

Iīm trying to write a program that captures everything that goes out
from the sound card (if your are listening a mp3, system sounds, etc.)

   There are multiple components that captures the sound from the mic
input. There are also a lot of components that write a PCM sequence to
the speakers, that transforms MP3 to PCM... But I canīt find one that
captures soundcard output.

   The more similar thing I have seen is one component called VUMeter,
but it doesnīt work in my D5 - Win2000 (itīs seems to be too old) and
one program of a spectrum analyzer... that analyzes the input, not the
output.

   I know this is possible since there are comercial programs that do
this.

   My knowledge of WinAPI is poor.

   Can anyone give me a hint?
