Still loaded with typos and mispellings (I'll get around to them), I
haven't completed the citations and appendixes, but the text is
available at <http://gagne.homedns.org/~tgagne/contrib/EarlyHistoryST.html>.

If someone has a copy of the PDF and can check my translations of some
of the programming examples, I'd appreciate it. Some were difficult for
me to decipher and I'm certain many are incorrect.

There are multiple issues raised in his paper that would be interesting
to discuss here.
