C# and survey software

Hello,
I am a web application developer and my company is currently looking
for a web app to create surveys at a resonable cost (we are a small
company). We must be able to have access to the data and link the
LDAP information to the survey information taken. (We are using Sql
Server (database), C# and LDAP Authentication) We also need it to
create reports both text based and visual (charts, etc...). We are
new to the C# and .net so any advice would be appreciated. Thanks so
