Without knowing what you have installed on your web server, which version of those web applications. Check the homepage of those applications you have and look for security bug information, could be that you have an application that is outdated with a serious bug, which is one of the most common ways to inject files.
Having globals enabled in php.ini makes it easier to inject files and of course if you use the same operating system as on the computer you posted your post from, then the operating system can be seen as a security risk too.
