Normally, that happens after someone has contributed to a project for a
while through patches, which gives the person a chance to show that they
know what they're doing, and do useful work without actually being able
to wreck anything.

There is a disconnect in that process if people don't get feedback on
their ideas - if they're good, they should get included, if not, the
problems should be explained, so that people understand what their
mistakes are and how to correct them.

For what it's worth, I do have commit access at this point, but it was
given to me to work on the Tcl Tutorial, not other parts of the system.
